Sports Physicals 5/4/21

The Chase County Clinic will be offering Wauneta Palisade students sports physicals:
When: Tuesday, May 4, 2021 from 9:00 am to 3:00 pm MT
Where: Chase County Clinic in Wauneta
Cost: $20.00 per student
Parents:
1. Complete the first page (front and back) of the Preparticipation Physical Evaluation form for each child participating in sports next year. (Form included with this note).
2. Send $20.00 and the above form with your child on the day of the sports physicals.
3. Expect a phone call from the Chase County Clinic (308-882-7299) to get parental permission to complete the sports physical.
While a sports physical is required, the physical does not have to be completed by the Chase County Clinic. This physical exam can be completed by the medical provider of your choice but must be done before your child is allowed to practice or play in games.
