Sports Physicals at School on May 12

Physicals for current 7-11th grade students will be offered on Thursday May 12th starting at 8:00 am mt at the Wauneta Attendance Center. Letters and forms are being sent home with students, any student that is planning on participating in any extracurricular activity must have a physical on file at the school. If you are wanting your student to participate in this please make sure all forms are filled out completely and payment of $20 (checks made out to Quality Urgent Care) are turned in to the Wauneta Office by Thursday morning.
